Use this online form to report a problem you had when trying to use your my pay card.

Provide the following information if you have experienced one of the problems listed below at a store, restaurant, hotel or other merchant location displaying my pay logo. Please note that if the business does not display my pay logo, it might not accept any my pay cards.

All fields are required unless labelled as optional. The information you enter will not be used for any purposes other than problem investigation.

To report any other issues encountered during a card transaction such as a dispute with a merchant, please contact the financial institution that issued your card by calling the number provided on the back of your card.

The details you submitted will be investigated to resolve this matter and to determine if there is any violation of the My Pay Rules.
